Motivation is provided to start or continue positive adjustments in lifestyle. Change can be difficult, but with a positive attitude you can do anything.
Recommendations are shared with you and your doctor. Using a team approach, we provide the latest information about diabetes treatment. Together we develop a nutrition plan tailored to meet your needs. Medications, monitoring systems, and other methods to control your diabetes are introduced or reviewed.
Support and extra care are offered at all times based on your special needs and concerns. It is common to have mixed emotions about diabetes such as anger, fear, or frustration. We encourage you to gather the support you need by involving your family and friends in your care.
Continuing Care is strongly recommended for positive health outcomes. Everyone with diabetes needs on-going education and regular evaluation of their diabetes management.
Small steps lead to success. We provide you with the important information. You have the power to use it. Diabetes is with you for life. So are we!
Life with diabetes can be challenging, but it is definitely manageable.
